NOTE: Submission of the registration form specifies your acknowledgement and agreement to:

• All MTBA and MSBL National Policies shown below and on our policy page ( Rules / Bylaws)

• All information provided in the form above is complete and true!

WAIVER

• 1. The risk of injury from the activities involved in this program is significant, including the potential for permanent paralysis

  2. and death, and while particular skills, equipment, and personal discipline may reduce this risk, the risk of serious injury

  3. does exist; and,

  4. I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM

  5. THE NEGLIGENCE OF THE RELEASES or others, and assume full responsibility for my participation; and,

  6. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe

  7. any unusual significant hazard during my presence or participation, I will remove myself from participation and bring such

  8. to the attention of the Company immediately; and,

  9. I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE,

  10. INDEMNIFY, AND HOLD HARMLESS MSBL/MABL, their officers, officials, agents and/or employees, other

  11. participants, sponsoring agencies, sponsors, advertisers, and, if applicable, owners and lessors of premises used for the

  12. activity ("Releasees"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss or damage to

  13. person or property associated with my presence or participation, WHETHER ARISING FROM THE NEGLIGENCE OF

  14. THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.

  15. I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT , FULLY

  16. UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY

  17. SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

CODE OF CONDUCT POLICIES

  • per the MSBL National organization

  • I will observe all rules as established by the Men’s Senior/Adult Baseball League’s Board of Directors at all times.

  • I understand that fighting, physical abuse of players, umpires, or spectators, and the use of abusive or offensive language will not be tolerated by the MSBL/MABL and violation of this rule could result in my banishment or suspension from the league and forfeiture of all fees paid.

  • I will observe all rules as established by the Men’s Senior/Adult Baseball League, Inc.

  • I understand that fighting, physical abuse of players, umpires, or spectators, and the use of abusive or offensive language will 

  • not be tolerated by the MSBL/MABL and violation of this rule could result in my banishment or suspension from the 

  • tournament and forfeiture of all fees paid.

  • I realize that Men’s Senior Baseball League, Inc. and facilities do NOT possess a defibrillator.

  • I realize that there is no guaranteed playing time on any given team, associated with regular season and tournament play. 

  • I certify that I am, or will turn 18 years of age this calendar year and/or 25 years of age and/or 35 years of age and/or 45 and/or 

  • and/or 50 and/or 55 and/or 60 years of age.

• I fully agree that the terms and conditions of this agreement are binding.

REGISTRATION AND FEE PAYMENT POLICIES

• • Absolutely NO refunds are available for the particular season under any circumstances (expect for in the case MTBA could not place a player on a team due to wait list circumstances).

  • No special placement requests will be accepted outside of those allowable via the online registration system as supported by posted team formation policies.

  • In the event of non-payment to MTBA regardless of reason, the player will be held in Bad Standing with MTBA and national MSBL association which will prohibit registration and participation in MTBA and alternative programs until such amounts owed are paid in full.

  • A charge of $25.00 will be added to all amounts owed for checks returned for insufficient funds.

  • Late charges will apply to any registration not fully paid by the deadline.
